Create Task Activity Entry

Add a manual log entry to a task's activity timeline.

Accepts JSON body: { "note": "...", "document_ids": [1, 2] } Creates a manual_entry event in task_activity_log.

Requires work_streams read (not write): logging activity on a task you can view is a floor-worker action, and the floor role only carries r. Cross-shop access is blocked by the task-scope assertion below.
