Get Avatar Upload Url

Presigned PUT for the caller's own profile picture.

Mirrors clients.get_client_logo_upload_url. Two-step by design: the browser PUTs the bytes straight to storage, then commits the path with the PATCH below, so a large image never travels through the API process.

IDENTITY COMES FROM THE SESSION, never from the body. The path is keyed on the caller's own user id, so there is no request shape that lets one person overwrite another's avatar.
