Retry Source Triage

Re-enqueue a fresh triage_source job when the latest run failed.

Refuses (409) when the latest triage run isn't in 'failed' state — caller would otherwise re-run a successful triage and produce duplicate pending rows (the facade's upsert dedupes by (source_id, extraction_id, gate_type, question_id), but the extra worker dispatch is wasted load).

Returns 404 when no triage run exists for the source (nothing to retry).
