Send Message Stream

Stream agent response via Server-Sent Events.

Uses LangGraph astream with subgraphs=True (plus messages, updates, custom) so nested conversation nodes still emit tokens and node_complete events. For shop, entity_id and thread are derived from session (shop_id + user + instance_id).
