Get Outreach Referral

One pending referral's own detail -- who named whom, at which account, and the sentence it came out of. fetch_pending only ever reads id off the queue list above; this is where the skill's task prompt gets the rest (org_id/from_contact_id) so it can call get_outreach_context. Filtered in Python over the same, deliberately small, pending list -- there is never more than a handful of open referrals at once (list_pending_referrals's own module: "the queue that makes the ladder move").
