Paced and, past issues_service.MAX_VERIFY_ATTEMPTS consecutive failures, escalated to a self-filed, buildable defect -- never a person's card, unless the failure names a genuinely external cause (an expired credential, an account out of credit), the one exception issues_service.record_verify_failed still raises a card for.
