What a reviewer's approval could not see: the customer said it made no sense, an agent had to walk something back, or it escalated into a bigger issue (docs/SUPERVISION_DESK.md "Post-send signals"). Gated on the SAME rooster:feedback:write grant as POST /feedback -- recording a post-send observation is the same class of human judgement, just arriving later.
