Refine Edit Backlog

The batch learn pass's read: every edit diff nobody has learned from, oldest first, keyset-paginated on (created_at, id).

Exists beside /refine/{skill_key}/evidence rather than replacing it. The evidence endpoint is the 15-minute heartbeat's 50-row steady-state read; this one exists to drain a 3,113-diff BACKLOG (the corrections accumulated over months while nothing read them -- verified in production 2026-09-03), which needs pagination and edits-only rows the heartbeat read was never shaped for.

after_created_at/after_id travel together: pass BOTH values from the last row of the previous page, or neither.
