Threads Uninstall Callback

Meta Uninstall (deauthorize) callback: the user removed our app.

The token is dead at this point, so the row is deleted rather than marked — leaving it would keep the refresh sweeper working on a grant that no longer exists, which is exactly the "latched account" failure mode.
