Update Account Voice Policy

Update the per-channel-account voice policy stored on channel_account.state JSONB.

Only the channel owner OR a member of any workspace the channel is shared into may flip the policy. The default ('approval') is the safe one; flipping to 'auto' opts the bot into entering group calls automatically when an incoming_call trigger matches.
