Regenerate Post Call Package Stage

Re-run one stage's generation with CURRENT upstream state (approved/ edited events; killed excluded) — clears the stage's stale flag, marks downstream stale (stage A regenerate stales both). Approved/killed markers on the regenerated stage reset; the apply_log is untouched.

SLOW: one full generation stage (30-120s) — async handler, no DB session held across the LLM await; use a generous client timeout.
