Skip Outbound Draft

Operator skip — the shared action (backend/outbound/actions.py), the SAME implementation the MCP tool calls: draft → skipped; in-thread kinds also file the thread no_action (a NUDGE skip is terminal forever); handoff kinds drop just the email; pinged threads get the cross-surface echo.
