Apply Post Call Package

Approve — the ONE boundary that can cross into the account book. Assembles the exact operations the selection implies and applies them per the package's apply_mode: sandbox records them (executed: false) and the book is untouched; live executes them through the internal app's own write paths with provenance. Already-approved events are idempotent no-ops (noted in apply_result.skipped); invalid/killed events reject with a 400 naming the event.
