List Schema Migrations

Return all migration files + applied state. Bootstraps the tracking table on first call so the page works on an unseeded DB.

target selects the database. KOLens has two: the app's own Postgres and Supabase (auth / profiles / credits / assistant). The Supabase tab needs SUPABASE_DB_URL; without it we return configured: false and an empty list rather than erroring, so the UI can render a setup hint.
