Set Desk Max Positions Per Symbol

POSITIONS PER SYMBOL — how many open positions the WHOLE DESK may hold on one name, counted by UNDERLYING (three NVDA contracts are three bets on NVDA). Five bots can all pile into the same name today and nothing notices. Null clears it. Refusals are skipped_symbol_concentration.
