Apply Review Fix

Commit one verified review fix onto its PR head branch (the in-app "Apply fix" button). The client sends only the run id + the fix's content id; the actual patch is re-loaded from the run's signed result server-side. REPOS_MANAGE-gated, paid + per-repo opt-in, behind the fix_apply_enabled kill switch; a typed error code drives the button's failure state (needs_approval / head_moved / fork_pr_unsupported / ...).
