Reconcile Autofix Runs

Drain the pending autofix outbox, sweep runs stranded by a dead worker, recover any drafted run whose review-trigger never landed (Gap A recovery), AND act on drafted fixes that went red (signal-when-red). All run whenever autofix is configured; a no-op (zeros) otherwise.
