Start Fv Run

An FV runner claiming its run: a lease, a read-only clone token, the tenant callback secret, and the TRUSTED verification policy.

Eligibility (entitlement, kill switch, fork) is re-checked inside start_fv; an ineligible or superseded run is terminalised there and this raises 409, so no expensive verification ever begins on a stale/unentitled run.
