Activate Sentry

Connects Sentry: exchanges the authorization_code for tokens, validates the installation, then bulk-deactivates any previously-active integration for the team and creates a brand-new row -- deactivate-then-create, NOT reactivate-in-place (design spec §6.3, a different shape from Trello's POST). client_secret is Shake's own platform-wide SENTRY_CLIENT_SECRET, never client-supplied.
