Activate Asana

Completes the Asana OAuth flow. Team-scoped, NOT app-scoped -- Flask's own route has no app_id segment at all (design spec §5.2), same shape as Notion/GitHub's activate; looks the pending row up by its unique state (including soft-deleted rows). Exchanges the code (with the PKCE code_verifier) for a refresh_token -- there is no access_token to store here at all.
