Activate Jira

Completes the Jira OAuth flow. Team-scoped, matching every prior tier's activate shape. Before bulk-deactivating other same-app Jira rows, best-effort deletes any of their live JiraWebhook subscriptions first (developer-approved, matching the Linear precedent -- Flask itself doesn't do this).
