Activate Notion

Completes the Notion OAuth flow. Team-scoped, NOT app-scoped -- Flask's own route has no app_id segment at all (design spec §5.2); looks the pending row up by its unique state (including soft-deleted rows), so the app is derived from the row itself, not from the URL. Exchanges the code via HTTP Basic Auth. Bulk-deactivates any other active Notion row for the SAME app (the row's own app_id) before activating this one.
