This is the pass decision, not just feedback. The practice case gates all real work, so "completed" had to stop meaning "clicked through": before this, a 0-of-4 stamped completed and that was the only thing between a new physician and a paid case.
Never touches the real submit pipeline: no submissions row, no records, no QA routing.
