Ingest Image Asset

Ingest a PNG/JPEG/PDF and attach it as a StudyAsset to a V4 (real de-identified) case's study (PRD §3). Enforces PNG/JPEG/PDF only (415), size/dim caps, PDF→raster, metadata strip, content-addressed store + dedupe. The image BYTES go to the asset store; only the reference is written to the case. Images NEVER enter V1/V2/V3.
