Remove inference provider access grant

Deletes exactly the selected grant and returns an empty 204; the legacy /access/{grantId} URL has the same behavior. Other grants and member credentials are retained, and OAuth callbacks recheck remaining access. Requires the Manage Gateway providers permission and enabled Gateway management; session callers must recently reauthenticate.
