Mark Account Intentional

Confirm an account's type is intentional (accountant).

Sets is_intentional=True on the account's ledger row in the open fiscal year that carries the code (one row; rows_updated is 1) so the accountant skips ACCOUNT_TYPE_INCONSISTENT for it. Idempotent; 404 if no open fiscal year carries the account. body.note is accepted (optional justification).
