Which held proposals an agent (MCP, API key, the AI chat) may accept (member, ZEN-2).

hold_causes empty = none (the default). ["low_confidence"] = invoice codings held only for low confidence, with no open question or risk flag (agent_acceptable: true in the decision list); the accept re-runs the duplicate, VAT and FX checks and refuses on a hit.
