List finished message decisions with one plain reason

Every decision that reached the end of the line, newest first, from the policy engine and from delivery requests. Decisions still waiting for approval or queued to send are left out. reason_label is the server's plain sentence; clients show it as is. Facts are provider and app facts about a sent message in time order; machine opens are never shown. Summary counts and top_reasons cover from, to, channel and engine but ignore outcome and reason, so each filter chip keeps its count. Keyset paging: pass next_cursor as after; it is null on the last page.
