9811d04c — remove a persisted custom plugin by name.

Name comes from ?name= or a {"name": ...} JSON body. ?hostname=X scopes to that machine. Built-in slot overrides are left untouched (only a non-built-in entry with a matching name is dropped). Returns the updated custom list. Removing a name that isn't a custom plugin is a no-op 404.
