REST mirror of the MCP record_handoff_correction tool — see meridian.handoff.record_handoff_correction / regenerate_handoff_correction for the full contract. Pass regenerate: true in the body to also repair pointers, invalidate the source handoff, and produce a new deterministic revision in this same call.
