b03be6a6 — serve the PWA service worker at the site root.

Root scope ("/") is required so the worker can control /dashboard. The Service-Worker-Allowed: / header is belt-and-suspenders; serving from / already yields root scope. Sent no-cache so the network-first SW itself is never pinned to a stale version.
