Create Api Token
Generate a new API bearer token for the authenticated tenant.
Requires a valid session cookie (browser flow) or existing bearer token. Returns {"token": "sk_meridian_...", "id": "...", "label": "..."} where token is shown exactly once and never stored in plain text.
