Trigger Test Error

Force an error response to exercise the 5xx counter + admin alerting.

Gated by MERIDIAN_ENABLE_ERROR_TEST=1 so it only exists on preview / staging — never on prod. kind=exception (default) raises an unhandled Exception so the middleware logs the traceback and synthesizes a 500. kind=500 returns a raw HTTPException(500) so the middleware records the response status without going through the exception path.
