Update Chapter Memory

Edit saved continuity memory for a chapter/revision.

This is a manual correction path for bad extracted memory. It replaces carry_forward_facts on the selected revision (latest by default). If the edited revision is the latest revision, the chapter-level memory is updated too so future prior-context ledgers stop carrying stale facts.
