Refresh Chapter Context

Regenerate continuity context for an existing chapter/revision.

Re-runs the same continuity extractor used after chapter writes and full chapter revisions. If the refreshed revision is the latest one, the chapter-level metadata is refreshed too so future context ledgers see the new facts.
