Load a saved chat thread for a chapter+revision.

Returns {job_id, chapter, revision, messages}. messages is a list of {role, content, plan?} entries in chronological order. Empty list if nothing was ever saved for this slot. Use revision=0 (or omit) for the pre-revision draft thread.
