Generate Chapter Cover

Generate a non-spoiler cover image for a chapter.

Reads the chapter, plans an establishing scene that sets the chapter up without revealing its climax or twist, grounds it on the featured cast's reference art (gpt-image-2 multi-reference edit — pose sheet preferred per character), and renders it through DesignForYou. Returns {cover_job_id, status: "running"} — poll get_chapter_cover_status until terminal. The generated cover is saved on the chapter (output_artifacts.chapters[n].cover).
