Approve Memory Proposal

Approve + apply one proposal atomically.

An interactive user may optionally submit edited_title and edited_body in the same request. The reviewed text and the applied text then commit in one transaction; process tokens may approve an already-reviewed payload (for the confirmed PI-assistant flow) but cannot rewrite it while approving.
