Get Process Tool Result

Full output of ONE tool call, fetched on demand from the owning process.

Newer cores sync only a bounded preview of each tool result (marked result_truncated, with its result_seq); the full text never enters the cloud copy. A phone that wants it asks here and the call is forwarded to the machine that ran the tool — nothing is cached on this side. Keyed on (seq, call_id) because provider call ids are not unique across a session. An older core without the route answers 404 (→ the UI says "update your workstation"); an offline process is the usual 409.
