List Process Conversations

Return only the conversation summaries (title / session_id / last_active). Per-conversation full body is fetched lazily on demand by get_process_conversation when the user actually opens one — eager-syncing every detail here used to cost N × RPC for every page load and was the main source of perceived "slowness" on the history panel.
