Create a dashboard

Create a dashboard. EVERY panel is validated: the pinned segment exists AND is visible to the caller, it is a BASE segment (derived segments are refused), the spec compiles against the grain's catalog, sql specs and artifact queries are execution-probed on the read-only role, and the artifact HTML is scanned for credentials. A 400 lists every failure at once under error.details.errors[] = { panel, message }. Ownership is stamped from the caller's JWT (null for a service call = org-visible), never from the body.
