Create a company relationship episode

Idempotent for an exact retry whose active child/parent/type/effectiveFrom match. A different overlapping start returns 409. A domain endpoint that does not exist is created as graph-only. Emits entity.relationship.created only when created=true, timestamped when the mutation occurred.
