List an element's revision history.
Returns the element's revisions newest-first (ADR-0021 Phase 3: per-level versioning). Each entry is a header — version, actor (with resolved display name + "via {key}" credential label), the optional edit reason, change summary, and timestamp — without the snapshot body. Use a version with POST .../restore to revert the element's content, or GET .../revisions/{version} to fetch a version's snapshot for a diff.
Keyset-paginated: pass limit (default 30) and, for older pages, the opaque cursor from a prior response's meta.nextCursor.

Query parameters
Page size. Default 30. An integer outside [1, 100] (or a non-integer) is rejected with 400.
Opaque keyset cursor from a prior response's meta.nextCursor. Omit to start from the newest revision. A malformed cursor is rejected with 400.
