Add Companion

Add a companion to this activity. Two modes:

  1. body.placeholder_id provided → link existing placeholder
  2. body.name provided → find-or-create placeholder by (owner, normalized phone) to avoid duplicates, then link

If channel='whatsapp' AND a phone is present, also build a wa.me URL and log a placeholder_invites row with status='draft'. The frontend opens the URL and PATCHes status to 'sent' on success.
