Mcp Sse Handshake

Open a long-lived SSE channel for the MCP http+sse transport.

Per the 2024-11-05 spec the first frame is an event: endpoint pointing the client at the POST URL it should send JSON-RPC to. The connection then stays open; POST /v1/mcp/messages enqueues JSON-RPC responses onto this stream as event: message frames.

Anonymous handshake is allowed when MCP_ANON_DISCOVERY_ENABLED is on — the client still has to pass a bearer token before tools/call will dispatch, but the SSE channel itself can be opened so discovery probes from MCP directories work without OAuth.
