Daily Signal Confirm

Consume a double-opt-in confirm link.

Hashes the token, matches a pending row, flips it to confirmed, then 302-redirects to the signal page with ?confirmed=1. An invalid/expired/consumed token redirects with ?confirmed=0 (no enumeration, no error page).
