Approve Mission

Approve a mission (optionally with an edited draft).

Approval fires the replied RL nudge inline via _record_outcome_once — symmetric with /actions/reject, which has always fired RL inline. The helper is idempotent, so the v1.5.x plugin's existing second /feedback/record call doesn't double-fire.

Called by the SignalPipe OpenClaw plugin and any direct integrations.
