V4 Ack Mission

v4 sender send-ack (§3.3) — replaces /actions/ack for daemon/plugin senders.

Path id + body { "outcome": "success"|"failed", "error_class": "...", "platform_response": "..." }. Shares _apply_mission_ack with the legacy endpoint, then adds sender-channel health signalling for the failure classes the platforms hand back.
