Book a claimed call prize into a reward slot

Changed on

Creates the call (and its GHL appointment on the coach's reward calendar) and assigns the coach. The optional focus and note go on the event and the appointment for the coach. 409 SLOT_UNAVAILABLE when the slot was just taken or is no longer offered — list again; 409 ALREADY_BOOKED when the prize has a live booking.
