Accept Invite

Accept a pending invite addressed to the caller (idempotent).

The gate reads the CALLER's account default, never the app owner's: the new membership lands on the caller's account, and the person who sent the invite must not hold the switch that protects the person who reads it. The route loads no App row for the gate, because a pending invitee cannot pass load_app_for.
